The NYSC ID Card (Sample, Application, and Replacement)

A few days before leaving the orientation camp, every corps member is issued a plastic identity card to use throughout their service year.

After your service year, you are to return this ID card to NYSC.

Your NYSC identification card is very important.

This card is required for your identification as a corps member anywhere in Nigeria and discharge certificate application and collection during your passing out parade.

Without it, you could face some avoidable stress down the line.

Every corps member shall during the service year carry on his person, his/her identity card wherever he/she goes and shall on request present it to the appropriate authority.

Any member who fails, refuses or neglects to produce his identity card on demand by NYSC official is liable to extension of service for a period not less than twenty one (21) days with half pay.

Where a member on leave requires medical attention, he/she shall report to a military or government hospital and shall be treated on production of his identity card.

Whether it is the temporary state code tag or the permanent identification card that you’ve lost, you can easily get a replacement.

Here are the steps to apply for and get a new NYSC ID card as a corps member in Nigeria:

1. Get a Police Report

Getting a police report first is important as it serves as proof that your card was lost and not stolen or used for something fraudulent.

To get a police report, look for the nearest police station that issues out such.

Then write a letter (accompanied by a photocopy of an I.D card such as voter’s card, driver’s license or national identity card) requesting for a police extract.

There are usually “business centers” around the police station that can help you with such letter.

2. Swear an Affidavit

After getting the police report, swearing affidavit for misplaced NYSC ID card is the next step.

Go to the nearest magistrate or high court to get an Affidavit of Loss.

An affidavit can only be sworn to at a court before a commissioner for oaths or a notary public.

The affidavit is a legal document where you’re swearing that you’ve lost your card.

Courts are usually pretty straightforward with affidavits, so this part shouldn’t take too long.

Just ensure all your details are correct before signing anything.

3. Visit the NYSC Secretariat

After getting your police report and affidavit, head over to your state’s NYSC secretariat.

When you get there, explain to them that you’ve lost your card and you need a replacement.

They’ll ask for the police report and the affidavit you got earlier.

Make photocopies and submit all necessary documents at the secretariat.

NYSC usually charges a small fee for replacing your ID card.

After making the payment, you’ll be given a receipt, which you’ll need to keep safe.

Once you’ve submitted everything you’ll have to wait for feedback.

NYSC will process your request, and eventually, you’ll get your new identification card soon.
